Do Good Multnomah is a non-profit organization that partners with the community to provide supportive housing and low-barrier emergency shelter to houseless veterans in Portland, Oregon.

While we are committed to providing participants with a safe, warm place to stay for the night, we are equally interested in forging meaningful relationships with them.
These bonds allow for our staff to serve as mediators and connectors between participants and a variety of service organizations in Portland.
Purpose of the role:
The PSH Case Manager is a member of the supportive services team whose mission is to support the Veteran residents at Clayton Mohr Commons retain and thrive in their housing.
Clayton Mohr Commons is a 24-unit, permanent supportive housing program, where each resident has a lease and engagement in services is voluntary.
The successful applicant will work with the CMC Peer Support Specialist and HUDVASH Social Workers to provide case management and referral services to residents, assisting them with goal identification and completion, tenant education, resource connection to address the eight dimensions of wellness, community integration, and eviction prevention.
This position will be a part of a small team who will support and engage in participants overall health and well-being using a strength-based, housing first and harm reductive model.
Applicants must be comfortable with working alone at least one day a week.
What you'll do:
Conduct case management visits in participants' homes and in the community to support, educate and engage participants in their own environment (often by oneself).
Work closely with the Veteran Peer Support Specialist to advocate, refer, and connect residents to community resources to ensure participants have access to a variety of services and benefits, including employment, mental health, and physical health services.
Assist participants with the development of participant driven individual goals to retain and thrive in housing.
Assist participants in understanding and upholding their lease and their tenant rights.
Coordinate with supportive services team, property management, participant support networks, and community providers to address the individual needs of each household and develop housing support plans for eviction prevention purposes.
Help support with admission and discharge planning for any participant needing hospitalization or sub-acute level of care.
Assist participants apply for eligible benefits, including Oregon Health Plan, food stamps, job training programs, etc.
Develop comprehensive knowledge of community and Veteran resources and maintain effective relationships with these resources to best meet participant needs.
Conduct program intakes, exits and follow-ups and ensure corresponding documentation is accurate and uniform.
Maintain accurate physical and electronic records, including utilizing the Homeless Management Information System (HMIS) and Case Worthy to enter and exit participants from the databases, and update information at least annually.
Prepare case notes and update service tracker to clearly document all engagement with participants.
Collaborate with Program Manager to host community building events and educational and skill development opportunities/programs for residents.
Work with the Program Manager to identify opportunities for volunteer involvement.
Attend case conferencing meetings with HUDVASH and supportive services team.
Maintain and advance professional knowledge/skills with the guidance of the Program Manager in order to drive continuous improvement in program effectiveness as measured by individual progress.
Attend all required trainings as instructed by Program Manager.
Perform all other appropriate duties as assigned.
Adhere to all state and federal privacy and security regulations applicable to the program, and to Do Good Multnomah's policies and agreements regarding confidentiality, privacy, and security.

Estimated Salary: $20 to $28 per hour based on qualifications.
